Named GP: As from June 2015 we are obliged to inform patients that they have a “named GP”. This has no bearing at all on which GP you currently see and / or wish to see in the future.

New Patient Registration

Surgery1.jpgIf you wish to join our practice and reside within our practice boundary (see below) you can do so be completing the Registration Form (GMS1) here. Alternatively, you can call into the Practice to complete these forms, or by requesting a registration pack via email, by emailing us STOCCG.MarpleCottageReception@nhs.net  A form will need to be completed for each person that wishes to register. If you have your medical card available please attach it to the form.

You may fill in the form straight away or take it home and return with it at a more convenient time.

The form will ask you for personal details such as your Name, Address, Date of Birth, Telephone Number, Previous Address and Previous Doctor. This information helps quickly trace your medical records and requests your records to be sent to the practice.

We will process your registration request usually within three working days, whereby after this time, you will be able to make appointments. 

On the back of your registration form you are given an option to go on a central register as a Donor.  However as from 1 June 2015 patients wishing to register for organ and blood donations need to self-register on the NHS Blood and Transport website www.nhsbt.nhs.uk  You can also do this via the NHS App.
